Generally when a hospice patient starts with our program, their physician consults with our hospice care team. This group consists of qualified physicians, nurse practitioners, nurses, social workers, chaplains, hospice aides, counselors, and volunteers.

Working closely together, team members treat and attend to the patient and family as the unit of care to provide exceptional care.
